Travel Insurance: Who Actually Needs It
Most short-trip travellers are told to buy cover they do not need. Here is the honest version.

For one week inside the EU with a valid GHIC or EHIC card, you probably do not need a policy. We would rather say that than sell you one.
When you genuinely should buy
Trips longer than about three weeks a year. Past that point a monthly subscription costs less than repeated single-trip policies.
Any non-EU passport, or travel outside the EU. Reciprocal healthcare stops applying and a hospital stay becomes a real financial risk.
Expensive equipment in your bag. A camera body worth more than your flights is worth insuring separately; most travel policies cap single-item claims well below that.
Anything involving altitude, water sports or motorbikes. These are the exclusions that catch people out. Read the activity list before the price.

Questions
Sometimes, and usually only if you paid for the trip with that card. Cover is often limited to trip cancellation rather than medical costs. Check the benefits guide rather than assuming.
It gives you state healthcare on the same terms as a local resident, which in Spain is good. It does not cover repatriation, lost baggage, or private treatment, and it is not valid outside participating countries.
Some subscription policies allow it, most single-trip policies do not. If you are already travelling, filter specifically for policies that permit mid-trip purchase.
